• Neurologic complications can follow primary EBV infection, although these are rare. should be considered as a precipitant of unexplained mononeuritis multiplex. serology requires repeating and careful interpretation to demonstrate evolution antibody responses confirm the diagnosis. A 20 year old lady presented acute medical unit with new neurological deficits. She described gastrointestinal symptoms in preceding 24 hours. Her case was discussed infectious diseases neurology overnight, both whom were resident at hospital on other side city. The team concerned enough discuss Public Health England for consideration botulism antitoxin. When this diagnosis discounted basis asymmetric deficits, patient admitted under care neurologists. Mononeuritis multiplex unclear aetiology diagnosed. subsequent review resulted likely trigger symptoms. Whilst neurologic deficits resolved, neuropathic pain continues. Learning points include importance an early, accurate description need consider Collaboration between disease physicians key managing disentangling broad array non-infectious differential diagnoses.
